Imitrex / Imigran (generic name: sumatriptan) is a medication primarily used for the acute treatment of migraine attacks, with or without aura, in adults. According to the FDA, it is not intended for the prophylactic therapy of migraines or for use in managing hemiplegic or basilar migraines.

Sumatriptan functions as a selective serotonin (5-HT1B/1D) receptor agonist, commonly referred to as a triptan. Its primary purpose is to alleviate migraine symptoms, including headache, nausea, vomiting, and sensitivity to light and sound. It is not used to prevent or decrease the number of migraine headaches a person experiences.

The exact mechanism by which Imitrex relieves migraine symptoms is not entirely understood. However, it is believed to work by stimulating serotonin receptors in the brain, leading to the constriction of dilated blood vessels. This vasoconstriction is thought to counteract the widening of blood vessels that occurs during a migraine attack, thereby reducing headache pain and associated symptoms.

Imitrex belongs to the pharmacological class of drugs known as triptans. Triptans are a group of serotonin receptor agonists that are specifically used to treat migraines and cluster headaches. They are effective in relieving migraine symptoms by targeting serotonin receptors to reduce inflammation and constrict blood vessels in the brain.

Dosage

For adults, the recommended oral dose of Imitrex is 25 mg, 50 mg, or 100 mg as prescribed, taken as a single dose. If the migraine symptoms persist or return after initial relief, a second dose may be taken at least 2 hours after the first dose. The maximum daily dose should not exceed 200 mg within a 24-hour period. ​ Imitrex tablets should be swallowed whole with water or another fluid. They can be taken with or without food. It is advisable to take the medication as soon as possible after the onset of migraine symptoms, although it is still effective if taken at a later stage during the migraine attack. Imitrex is intended for use on an as-needed basis for the acute treatment of migraines and is not prescribed as a regular daily medication. Therefore, the concept of a “missed dose” does not apply. Patients should take Imitrex only when they experience a migraine attack, following the dosing guidelines provided by their healthcare provider.

Storage

Imitrex tablets should be stored at room temperature, between 20°C and 25°C (68°F and 77°F).

Is sumatriptan safe for everyone?
No, sumatriptan is not recommended for people with heart disease, uncontrolled hypertension, or certain neurological conditions. Always consult a doctor before use.
Does Imitrex work for all types of headaches?
No, Imitrex is specifically for migraines and is not effective for tension headaches, cluster headaches (unless prescribed off-label), or other headache disorders.
Does Imitrex have potential for addiction or misuse?
No, Imitrex is not a narcotic or habit-forming drug. However, overuse can lead to medication-overuse headaches, so it should be used only as directed.

Most people who use Imitrex do not experience any adverse side effects. Doctors prescribe this medication because they assess the benefits of such treatment outweigh any likely unwanted effects.

If a sudden allergic reaction (anaphylaxis) occurs after taking Imitrex, with symptoms like swelling of the face, tongue, or throat making it difficult to breathe or swallow, or there is wheezing, hives, rash, blistering, or peeling of the skin, call a doctor or 911 right away, or go to an emergency room immediately.

Some of the side effects that have been reported include pain or tightness in the chest or throat, tingling, flushing, weakness, dizziness, abdominal discomfort, sweating, nasal irritation, and injection site reactions. These effects are generally transient and mild to moderate in intensity.

Conditions treated by Imitrex

FDA-approved uses: Imitrex is approved for the acute treatment of migraine attacks with or without aura in adults. Migraines are characterized by severe, throbbing headaches, often accompanied by nausea, vomiting, and sensitivity to light and sound. Imitrex is effective in relieving these symptoms during an active migraine episode.

Off-label uses: In some cases, Imitrex has been used off-label for the treatment of cluster headaches, which are severe headaches that occur in cyclical patterns or clusters. However, its safety and effectiveness for this use have not been established, and it is not FDA-approved for this indication.

  • What does Imitrex do to you?
    Imitrex or Imigram (sumatriptan) are selective serotonin receptor agonists. These work by binding selectively to vascular receptors, producing a vasoconstrictive effect on cranial blood vessels. This helps to block pain signals from being sent to the brain, as well as the release of substances that cause pain, nausea, and other symptoms of migraine.
  • Can I drink alcohol with sumatriptan?
    Evidence shows that alcohol can trigger migraines and cluster headaches. However, alcohol does not affect how sumatriptan works. Most doctors advise that it's best not to drink alcohol during a migraine attack.
  • How addictive is sumatriptan?
    Studies have determined that sumatriptan has low abuse potential and does not cause feelings of being high. As well, the substance is not a cause of physical dependence.
  • Can you take sumatriptan if you already have a migraine?
    Most doctors advise that Imigram should only be taken when the migraine symptoms start developing instead of when there is a sense that a migraine may develop. It should not be taken at the same time as other migraine treatments, such as other triptans or ergotamine.
  • Can I take Imitrex with ibuprofen?
    If the initial dose of sumatriptan does not work quickly to relieve a migraine, consult with a doctor about taking a painkiller containing aspirin, paracetamol, or a non-steroidal anti-inflammatory drug (NSAID), such as ibuprofen.
  • Is sumatriptan a strong painkiller?
    According to published guidelines, Imitrex is not a painkiller. However, doctors can prescribe it if painkillers or non-steroidal anti-inflammatory drugs (NSAIDs) have not worked. Sumatriptan comes in three forms (as tablets, a nasal spray or injectable). The nasal spray and injections are usually used to treat rapid-onset migraines and cluster headaches.
